To enter, all you need to do is fill in the online entry form and explain in up to 1,000 words what you did and why, and the benefits for employees and the organisation.
There is no fee for entering.
The completed forms and entry must reach us by Friday 14th June 2013.

2 representatives from each of the category winners will be invited to the Personnel Today Awards on the night of the 18th November in London.
The 'Best of the Best' will be announced during the Personnel Today Awards and presented with their trophy on the night.
You will be able to start your entry, save it and return at a later date.
Judging process:
The shortlist and winners of the Occupational Health Awards are chosen by respected individuals in the field who are completely impartial.
There is one judge for each category who selects a shortlist of three and later an overall winner.
The judges will assess the evidence of how you/your team achieved excellence in the following areas:
• Innovation and achievement
• Teamwork (including communication with partners outside the department)
• Leadership
• Effective use of resources
• Impact on employees and the business / organisation
